OCA templates

Use the following summaries to help you select the pre-existing OCA survey that best meets your needs.

'One-minute' survey
For daily feedback, use this short 3-question survey to ask students to write the most important thing they learned in class that day and to list any additional questions and comments they may have.
Class reaction survey
For daily feedback, use this 13-question survey to assess your performance that day. Students provide ratings of several aspects of instruction and an overall rating of class that day. Two open-ended questions ask students to explain their ratings.
Instructor evaluation
This 13-question survey allows you to obtain feedback about how effectively you are presenting material over a longer period of time. Open-ended questions ask students to explain what has helped and hindered them in the course, and to provide suggestions to improve instruction.
Ongoing course feedback-1
More comprehensive and extensive, use this 26-question survey to assess major course issues such as student preparedness and progress, the atmosphere of the class, and your communication and availability. Particularly useful if administered midway through a course, the survey can guide you in making mid-term adjustments. Open-ended questions enable students to explain answers to some scaled questions and to offer additional comments.
Ongoing course feedback-2
This 22-question survey primarily evaluates instructor behavior but also contains five student demographic questions. Particularly useful if administered midway through a course, the survey can guide you in making mid-term adjustments. Open-ended questions ask what has helped a student most, what has interfered with learning, and what changes should be made.
Teaching Analysis By Students (TABS)
This extensive, 29-question survey provides excellent feedback on instructor performance, with a response scale ranging from "no improvement needed" to "considerable improvement needed." Open-ended questions ask students to explain some ratings. There are also five long-answer questions.
Short TABS
This shortened 10-question version of the TABS focuses on preparation, presentation skills, communication of expectations, and responsiveness to questions. Open-ended questions ask students to provide suggestions and to explain what has most helped and hindered learning.
