Procedures for Conducting a
Course-Instructor Survey




  • It is the policy of the Course-Instructor Survey Division that the evaluations remain anonymous. Neither the Instructor nor the T.A. is to be present during the evaluation, and neither is to see the answer sheets until grades have been officially reported. Therefore, you will need to select a student in each class to administer the survey. Please remember to notify each Student Administrator of the date on which they should conduct the survey.

  • Each person signed up for a survey (with the exception of those planning early surveys) will receive written instructions shortly before the scheduled survey period.

  • Instructions on conducting the Course-Instructor Survey will be included in the survey packet for the Student Administrator.

  • Plan to allow 20-30 minutes of class time for the survey.

  • If your class is taught by more than one person (e.g., team taught, TA), the students must be sure which individual they are to evaluate. The Student Administrator should make this point clear.

  • Each instructor of a team-taught course, and both the instructor and the TA, or AI can be evaluated. However, each person must have a survey request made through the CIS office, and each person must have a separate survey conducted in his/her name.

  • If more than one survey will be conducted during one class period, please have one survey administered and collected before the second survey is begun. Each survey's answer sheets must be returned in its proper envelope.

  • Conducting an Early Survey is an option. Any survey scheduled before the survey period is considered early. Early surveys must be requested directly from the CIS Office. Early survey requests cannot be made through the eCIS system. Instructors should inform their department of their early survey requests. The department person will contact the Course-Instructor Survey office for early surveys. The surveys will be delivered to the department office. The department staff will distribute the surveys to the instructor.

  • Survey results will be sent to you 3-4 weeks after the end of the survey period.

  • If you would like additional information not covered by the items on the Course-Instructor Survey questionnaire you will use, there are two options available to you:

    A. You may include a few multiple-choice items of your own. Your additional multiple-choice questions can have a range of two to five choices. There is room for 20 items on the answer sheet. For a list of supplemental questions that may help you choose additional items, see supplemental questions.

    B. You may ask a few essay type questions, and instruct the students to write their answers in the comments section of the answer sheet.

    C. Supply the student Administrator with enough copies of the additional questions for your class, or instruct the Student Administrator to write the questions on the board. When you receive the computer-printed summary of your results, you will need a copy of these questions to interpret the results.
