Questionnaire Options by College

Teaching Assistants will not automatically be signed up for a specific form. From the eCIS main menu, choose Add A Survey Record to add TAs. You will need the TAs University EID, and class unique number to create a record, and you will need to make a questionnaire choice. You may choose the Teaching Assistant Form, which is designed for non-teaching TAs or, if appropriate, another approved form (such as lab or discussion).
Please note also that it is the expectation of the Provost that all faculty members who are participating in a merit review, are being considered for promotion or tenure, or are involved in post-tenure review will have their teaching evaluated through the Course Instructor Survey.
Nine of the items on the Basic Form will be released on-line. The same nine items are repeated on the Expanded Form, and they will also be released when the Expanded Form is used.
