How to Arrange a Course-Instructor Survey

To access the request system, go to https://utdirect.utexas.edu/diia/ecis/admin/ and click on "Search for Survey". To request electronic CIS, please email instructor EID, course unique number, and form preference to cis@utlists.utexas.edu .

Instructions for participating in the Course-Instructor Survey vary depending on college requirements. Instructors in departments that have a questionnaire choice should contact their department office with their selection.

For the name of the contact in your department, email the CIS office at cis@utlists.utexas.edu (preferred), or call us at 471-8175.

The CIS department contact will make the appropriate questionnaire selections on the survey request system. Only the department contacts are authorized to make these selections. For instructions on the use of the survey request system, see Instructions For Course-Instructor Survey Requests.
