This form is for registrants paying with MasterCard, Visa, Discover, or American Express only. For organizations that wish to register multiple participants for the classes, please fill out the top portion of this form with your organization’s address and contact information, and also complete the second section with the names and class date.
All ITC classes are $600 per registrant as of October 1, 2006.
