CASA Transcript Request

CASA Alumni may request transcripts from the CASA administrative office in the U.S. There are a number of variables that affect the length of time required to process your request, so please allow a minimum of 2-3 weeks.

Please provide the following information, in typewritten form:

1. The year of your CASA fellowship
2. Your name
3. Your address
4. Your phone number
5. Your email address
6. The number of transcripts you are requesting
7. The name(s) and address(es) where the transcripts should be sent

Transcripts cost $10 each. Checks should be made payable to "CASA/UT-AUSTIN."

Please email the form and mail the form, along with a check to:
