Longhorn Link Program Application

Longhorn Link (Student Support Services) is a comprehensive program designed to assist you in successfully completing your academic goals and degree throughout your college career. The information on this form is required for our federally funded program and is strictly confidential; it will only be used to determine eligibility.

Longhorn Link is required to submit an Annual Performance Report to the U.S. Department of Education; information from this application will be used. This report will also include participants' Social Security Numbers (SSN). Although SSN are not required to receive services, the U.S. Department of Education does request that information. In compliance with The University of Texas at Austin policies and procedures; however, the Longhorn Link Program will not request your SSN from you but will instead use confidential internal means to obtain that information. If you do not want the Longhorn Link Annual Performance Report to use your SSN, a written request must be submitted to the program.

I hereby certify that the above information is true and correct to the best of my knowledge. I further authorize Longhorn Link (Student Support Services) to receive copies of my high school transcripts, SAT/ACT scores UT Austin grade reports, financial aid award notices, and any other information from my academic records pertaining to my enrollment in Longhorn Link.
