Sign Language Interpreter/Captioning Services Request

This form is for requesting a sign language interpreter or captioning services for departments and/or campus events. If you are an enrolled UT student requesting service, please use the Interpreter Captioning Request Form for Students. All information requested on this form is necessary to secure services.

To secure services, this form should be submitted as far in advance as possible, but at least three business days in advance of the requested date of service.  Last minute requests cannot be guaranteed, though attempts will always be made to secure interpreting/captioning services. If you cancel services, you must do so at least 24 hours in advance to avoid any charges. Email Lauren Kinast (lkinast@austin.utexas.edu) if you have any questions about the request or the need to cancel services.

Funding for interpreting/captioning services for university faculty and staff will be provided centrally from September 1, 2010 through August 31, 2011.  Centralized funding for events open to the public will be reviewed and centrally funded on a case by case basis.  For questions, please contact Linda Millstone, Associate Vice President (471-2437).
