Accommodation Letter Request
Steps for notifying your instructors of your approved academic accommodations:
- Fill out and submit this form.
- Come by SSD to pick up your letters. (The letters will generally be ready within 3-5 business days from the request.)
- Once you obtain your letters from SSD, meet with your instructors to give them the letters and discuss your accommodations.
SCHOOL OF LAW: ONLY request a letter for Brandi Welch.
COLLEGE OF PHARMACY: ALSO request a letter for Dr. Richard Wilcox.
SCHOOL OF NURSING: ALSO request a letter for Dr. Linda Carpenter.
PLACEMENT EXAMS: request a letter for Center for Teaching and Learning.
