Work-related Injuries

Employees are to report their injuries immediately to their immediate supervisor. If the injured employee requires prompt medical treatment, the supervisor notifies Personnel Services immediately in the event the treatment provider calls for verification.  Supervisors complete and sign the Injury Report Form. The employee also signs the form, which is then sent to Personnel Services via the supervisor’s/employee’s reporting unit.
