When is Evidence of Insurability Required for Retirees?

Evidence of Insurability (EOI) is the record of a person's past and current health events and is used by the insurance company to verify if a person meets the definition of good health.

Medical or Term Life EOI forms should be completed online using My UT Benefits (MyUTB, formerly UT Touch) online enrollment system. All EOI applications, both online and paper, must be completed and submitted either electronically through MyUTB or postmarked by US Mail no later than August 15, 2009. If you are adding UT Select for yourself or a dependent and have other group health insurance, you must submit proof of the other coverage to the Human Resource Service Center by August 15, 2009.
