Performance Management: Performance Evaluation Form and Instructions

The form approved for use in the university's Performance Evaluation process comes in two formats, paper (available through University Supply and Mail) and electronic template form, which is available on this Web page. The template version of the form was designed to be completed on your computer, using Microsoft Word 97/98 or higher. It is part of a downloadable folder, which includes additional information to assist you in using the form.

1. Instructions
This document is the single most important key to successful use of the form. It provides essential guidelines for use of the form, including detailed examples. It will be especially valuable to those unfamiliar with using tables in Microsoft Word. Please take a few minutes to review it. You can also view the Instructions online.

2. Evaluation Form
This file is the actual Performance Evaluation Form that has been saved as a "stationery," or "template" document. Each time this file is opened, it will create a new, untitled document.
