All automated workers' compensation forms are housed on the mainframe in the *WCMENU application. If you're a departmental HR representative, you're eligible to gain authorization for *WCMENU.

How to Gain *WCMENU Authorization

If you want to gain access to the *WCMENU, have your department director/chair, assistant director, or CFO send a written request to the HRS workers' compensation manager. Make sure the person writing the request provides your name, acknowledges that you are a HR representative, and includes your *DPUSER ID. This process could take a while, so you shouldn't go through this process in the event of a workers' comp incident—you should have your department's WCI representative complete the form instead.
