Insurance extension for Optional Practical Training/Academic Training

It is possible to extend insurance coverage during Optional Practical Training.

The following guidelines must be followed:

1. Student must have purchased Student Insurance coverage the semester previous to OPT.
2. Extension must be processed during the 30 day period after final day of enrollment period for the previous semester.
3. Student is responsible for contacting Student Insurance to enroll. Student Insurance Customer service: (1-888-344-6105). Ask for "Enrollment". **
4. It is also possible to download the enrollment form and mail it in to the insurance company with the proper payment. You can purchase one month at a time but once you max out the six months coverage, there will be no more coverage benefits. Please follow the instruction on the form for payment and mailing.”
5. There is a ONE TIME purchase option for a maximum of 6 months coverage.
