Grade Report Authorization Form

All students must submit a Grade Report Authorization Form to have an unofficial grade report sent to their sponsor. Please use the link above to submit a Grade Report Authorization Form.

Students do not need to submit the form more than one time; however, students must submit a request each semester they would like the grade report sent.
