Ordering: Telephone Desksets
Fill out and submit the Voice Line and Telephone Instrument Service Request to ITS for installation or a quote. This form is used for both ordering a new deskset and new service.
Note: When ordering Meridian M5316 phones, please attach a face sheet to your request. Face sheets are templates of the various telephones and add-on modules that we provide and support (MBS II phones). The primary telephone line should be placed on Key 1 at the lower right hand corner of the set. For new lines where you do not know the phone number, enter "new."
