Lost/Stolen ID Cards
Individuals who have lost their university ID card or have had it stolen should deactivate their card IMMEDIATELY as follows:
- A regular university ID card may be deactivated at any time using the Lost/Stolen ID Deactivation page.
- For lost or stolen proximity ID cards, please follow the lost proximity card process. If you have proximity card access to a highly secured area, notify the UT Police Department (512) 471-4441 immediately.
A replacement ID card may be obtained at the ID Center during normal office hours. Government-issued photo identification is required. A $10 fee will be charged for all replacement cards.
Note: If you find your ID card after it has been deactivated, you must come to the ID Center to have it reactivated.
