Checklist for New Hourly Employees
Note: Some of the forms on this page require Adobe Reader or compatible software
Student Employees must complete all of these forms before they begin working.
- Complete the top part of the School of Law appointment form (PDF), print it, have Department Head or Professor complete the bottom half and sign it. This form must be re-submitted each semester, prior to starting employment.
- Print & complete the Background Check form (PDF).
If you have worked for UT within the past 12 months, you need only submit these two forms.
If you have not worked for UT in the last year, then you are required to also submit the following documents:
- Complete the Payroll Services Employee Information Form (PDF) or if you have worked for UT at any time in the past, you may complete it online at My Paycheck Profile under BOTH “My Bank Info” & “My Paycheck Distribution”. Submit a voided check with the form.
- Print & complete the W-4 Form (PDF), or if you have worked for UT at any time in the past, you may update & complete it online at My Paycheck Profile under “My Withholding”.
- Complete the I-9, Employment Eligibility Verification Form (PDF). You must bring the forms of identity along with the I-9. This form and required documents must be completed and turned in within two days of starting employment or Payroll will hold your paycheck.
- Print & complete the Ethics Statement (PDF), or if you have worked for UT at any time in the past, you may update & sign it on-line.
- Print & complete the Veteran Status Form (PDF), or if you have worked for UT at any time in the past, you may update & sign it on-line.
- If you are a male, print, complete, sign the Selective Service Eligibility & Verification Form (PDF) and the online Verification of Registration Form.
- If you are on work study, please provide your award information and the dates that you are authorized to work.
Once all of these are completed, please return them to Human Resources in 2.118B.
