Instructions: Once each academic year, each student at the LBJ School of Public Affairs is required to review the University and LBJ School policy on academic integrity and professionalism. This may be accomplished by completing each of the following:

1. Complete all modules of the UT Graduate School Ethics Training.

2. Review LBJ School presentation on integrity and professional conduct

3a. Review the Dean of Student's material on academic integrity.
or
3b.
Read the "Guide to Avoiding Plagiarism" published by the LBJ School Writing Center.

4. Review the Student's Guide to Sex Discrimination, Sexual Harassment, and Sexual Misconduct and the Nondiscrimination Policy published by the Dean of Student's Office.

Download and sign the Acknowledgement Form. (Your signature indicates that you have read and/or reviewed the materials specified above, in the current academic year.) When signed, drop the form in the Advising Forms box outside of OSAP. It will be filed in your records. You may also fax your signed form to L. Neighbors at (512) 471-1835.

Your Form must be complete and current (i.e., signed in the current academic year), in order to register for classes for the next semester.
