(1) Filing of this form enables UT to waive a fee assessment when you apply for readmission. Failure to complete and file the form will result in an assessment of a fee when you reapply.
(2) Fill out the information including your major code. (Link for all major codes)
(3) Place signed form in forms drop-box outside of OSAP.
(4) A copy of the signed form will be returned to your mailbox.
(5) When you are ready to resume classes, you must apply for readmission. |
