Annual PhD Student Progression Worksheet
The Graduate School directs all programs to periodically review each doctoral student’s progression toward degree completion. Because the School of Nursing is a professional school, we also include mentoring in professional skills and activities that facilitate professional career development in this advisement process.
The Annual PhD Student Progression Summary process is linked to Spring advising each year. The directions are as follows:
Student Responsibilities prior to Spring Advising:
- Download and complete the Annual PhD Student Progression Worksheet (PDF Format).
- Update your Curriculum Vitae.
- Pick up your Program of Work, unofficial transcript, and prior Progression Summaries from the Graduate Student Affairs Office (NUR 2.104L).
- Make an appointment for spring advising with your Faculty Advisor and bring the items in #1 – 3 to the advising appointment. (In the case of distance advising, you should make these documents available to your Faculty Advisor through prior arrangements.)
- Return all documents including the completed Annual PhD Student Progression Worksheet and the Faculty Advisor Summary Form to the Graduate Student Affairs Office.
The Graduate Student Affairs Office must receive completed and signed forms prior to lifting your registration bar.
