The application form is composed of four pages, as well as one copy of the FRA Request for Reference Form (.pdf) and the SRA Request for Reference Form (.pdf). Other materials must be added to the form to make up the complete application. Go to the link "Preparing the Application" for complete instructions for preparing and assembling your application. There are links within the application to access the instructions.
You may complete the application using the individual forms for the pages which are listed below in .pdf format. To access them you will need Adobe Acrobat Reader 7.0 or newer installed on your computer. Be sure to set your browser preferences to access .pdf documents using the Acrobat software you install. Acrobat Reader allows you to complete the forms online and save the documents to your desktop. It also allows you to partially complete a form, save it, and then make changes as needed. If you are using an older version of Acrobat Reader it is highly recommended that you upgrade your version. If not, you may need to complete the form online and print it if the version you are using doesn't allow you to save the completed form.
The complete application packets, which include instructions and one copy of the application in .pdf format, are available for printing only (FRA application packet - .pdf; SRA application packet - .pdf). The complete packets are not forms and cannot be filled out on your computer.
If you have difficulty accessing or using the forms online you can request a printed version of the application from Maggie Keeshen in the Faculty Development Program office (mkeeshen@mail.utexas.edu or call 232-3608). Printed applications may also be picked up in the Graduate School, Main Building 101, Monday through Friday, 8:00 a.m. - 3:30 p.m.
Each of the following pages must be included in your application with the exception of page 3, which you may omit if you answered "no" on page 1 to, "Have you held previous SRAs, FRAs, or Dean's Fellowships?" For instructions on completing these pages please refer to "Preparing the FRA/SRA Application."

The top section of the FRA and SRA Request for Reference form should be filled out by the applicant, and then forwarded to the recommender. The recommender should complete the lower portion, attach his or her letter, and send it directly to the Faculty Development Program office address or fax number on the form.
