This list of forms includes items on the student forms page but also includes the fellowship forms that students do not fill out. They are for use by graduate advisers and graduate coordinators only.

- Doctoral students (IMPORTANT: Carefully read and print both the Graduation Deadlines and the Checklist to be sure you prepare all the necessary items required for submission of your dissertation or treatise to the Graduate School.)

The following is a master list of links to forms you may need access to while enrolled as a graduate student at The University of Texas at Austin. If you don't find a form listed here it is possible that the policy requires submitting a petition or letter with required signatures. - Online Doctoral Degree Candidate Form - Required for filing to graduate. Complete by published deadline during the semester you plan to graduate.

- Request for the Final Oral (required 2 weeks prior to the defense)

Master's Students

- Program of Work - Effective Fall 2008, an electronic Program of Work that has been approved by the Graduate Adviser will be reinstated as a graduation requirement for all Master's Degree Candidates. This paper form is for use by students enrolled in 698A or by special permission of the Graduate Dean. Dual-degree students must use the Dual-Degree Program of Work.
