Educational opportunities for insurance options set

Evidence of Insurability (EOI) forms are now being accepted. All completed EOI's must be returned to the Insurance and Retirement Group no later 5 p.m. on July 31.

An EOI is a short form that an employee or dependent completes to answer brief questions about the employee or dependent's medical history. Employees do not have to get a physical before completing the form. Insurance choices that require an EOI include:

  • Adding a spouse or dependent children to the UT Select Plan or the United Healthcare Plan if they are not covered by a group health plan elsewhere. If your spouse or dependent children are currently covered under another group health plan and you wish to add them to your UT health plan, you will need to provide evidence that the person has current coverage, or the EOI will be required. EOI will not be required for adding a spouse or dependent to Humana HMO or to Prudential HMO.
  • Enrolling in long-term disability insurance
  • Enrolling in or adding a dependent to your existing term life insurance
  • Increasing a multiple of your term life insurance (for example, one to two times annual salary)

Evidence of Insurability forms can be found on the web at www.utsystem.edu/egi, from the Office of Human Resources, 600 W. 24th St., or by calling 471-4343. The completed forms also must be returned to the Insurance and Retirement Group at J5600. If employees mail the forms directly to the insurance company, it could cause a delay in processing.

Educational Opportunities
The Insurance and Retirement Group will be sponsoring the following educational opportunities before and during the July Annual Enrollment period. The group encourages employees to take advantage of this opportunity to learn about their insurance options for the upcoming year.

Benefits Fairs
Benefits Fairs are the opportunity for employees to meet representatives from the various insurance companies. Each representative will be giving a short presentation between the hours of 11:30 am and 1 pm.

Orientation Sessions
Orientation Sessions are conducted by members of the Insurance and Retirement Group. The format of these sessions is a brief overview of the insurance plans, the changes for the coming year, and important reminders.
