All samples should be accompanied by a Work Order Form, including a valid account number for billing. Users submitting samples to PHR 1.110
should fill in the computer sample log directly with the correct account number.
CRED members must fill out the CRED project description form in addition to submitting a CRED service request via the online MDAnderson database
system.
