HUB Forms

The following forms may be useful when contracting with the university:

• Prime Contractor Bid List Request Form (pdf)
• Professional Services Consideration Request Form (pdf)
• HUB Plan Form (Word, 806kb)
• HUB Subcontracting Plan Progress Report Form (Excel, 1.3MB): If you are awarded a contract, you must complete this form and submit it with each payment application.
• Transmittal Letter and HUB Subcontracting Plan for Competitive Sealed Proposal or Conventional Bid (Word): If subcontracting opportunities are probable, each respondent must complete sections 1-11 of this packet.
• Commitment Letter and HUB Subcontracting Plan for Construction Manager at Risk, and Design Build (Word): If subcontracting opportunities are probable and the respondent is able to execute the project with its own resources, each respondent must complete Sections 1-2 and Sections 9-11 of this package.
• Commitment Letter and Letter of “Good Faith” for Job Order Contracting and On-call Service (Word): Since subcontracting opportunities are unknown at the onset of this delivery method, each respondent must complete the commitment letter and prepare an additional letter of “good faith” that describes a procedure or methodology for HUB utilization on all job orders. This applies to project assignments more than or less than $100,000, and the awarded respondent shall submit a complete subcontracting plan for all assignments more than $100,000 or submit the subcontract form for all projects less than $100,000.
• Texas Minority Organizations Contact List (Word): This is a list of minority associations and other organizations that can give you access to plans, specifications and other details about the university’s contracting and subcontracting opportunities.
