Administrative officials and appointing officers are reminded that personnel forms (request for appointment and request for change of status) prepared for persons who hold academic rank and who are also nominated to additional part-time nonteaching positions must bear the approving signatures of both the individual's departmental chairman and his academic dean in addition to the project director before the forms are submitted to the Office of Personnel Services and Employee Relations.
