Faculty of all ranks, including Lecturer, Specialist, Adjunct, Clinical, Research Professor, are responsible for completing a Faculty Annual Report (FAR) of their academic activities each year. The FAR is an essential component of the annual performance review and evaluation of a faculty member's service as required by the UT Austin Handbook of Operating Procedures §3.17. The FAR is also an essential component of materials compiled for third-year reviews of tenure-track faculty, promotion reviews (all ranks), and periodic reviews of tenured faculty. Each member of the faculty should fill out the report with care and submit it to the departmental chair/director on or before Monday, October 1, 2007. A copy of report is included in each faculty member's personnel file at each administrative level.

To assist faculty members with completion of the Faculty Annual Report, the Provost's Office distributes the following reports to the departments and academic programs each June, via the dean's office:
• Instructional Activity Report (Fall 2006 and Spring 2007).

A summary of courses taught during the Summer Session will be distributed
in August.
• Report of Patents Issued (through May 31, 2007).

This report is provided only for those faculty members listed as
inventors on patents for which application was made by the University.
• Report of Active Projects (through May 31, 2007).

Information in the report is from the Grant/Contract database maintained
by the Office of Sponsored Project
• Report of Thesis/Report/Treatise/Dissertation Supervision, Committees, and Degrees Awarded.
Those needing fall and spring information before August can access this information through the mainframe application *GSADM. For authorization contact John Dalton in the Graduate School at 232-3626, or an email to jdalton@mail.utexas.edu.
