Authorization of Individual/Professional Services Form (AIS/APS)
The title of the web form has been changed from APS (Authorization of Professional Services) to AIS (Authorization of Individual Services), but the manual/paper form retains the APS designation. The electronic document ID authorizing these services is referred to as a PBS document.
