The Office of Research Support (ORS) does not administer or oversee HOP policy 5-2011 or training related to the policy. ORS administers HOP policy 7-1210, “Promoting Objectivity in Research by Managing, Reducing or Eliminating Financial Conflicts of Interest” so if you conduct research and have:

  1. Not submitted a Financial Interest Disclosure (FID), you will need to complete mandatory training and file a FID. Instructions for completing these requirements are located at http://www.utexas.edu/research/rsc/coi/training.html.
  2. Previously submitted a FID, you are not required to complete additional training nor re-disclose the same information to comply with the UTS 180/HOP 5-2011 policy requirements.   However, you may have other responsibilities under that policy.

How to Submit your Financial Interest Disclosure (FID) Form

Federal Regulations and University policy require the completion of Financial Conflict of Interest (FCOI) training and submission of a Financial Interest Disclosure (FID) form by covered individuals.

The Covered Individual must complete the Financial Conflicts of Interest (FCOI) training (Module ID: RC 410) prior to submitting a FID form. FCOI training records are updated one day after the training course is completed by the Covered Individual. The FID software, eProtocol, will not allow submission of the FID until the training record is updated.

The Covered Individual will receive an e-mail reminder regarding submission of the FID form when the training record is updated in eProtocol.

NOTE: You cannot submit your FID form on the same day you complete your FCOI training because of the training record delay. On the day after you complete the training, follow the instructions below to “Submit a FID Form that You Previously Edited and Saved.”

Instructions for Submitting a FID Form

Firefox is the recommended web-browser when using eProtocol. You will need to allow pop-up windows for “spike.orsc.utexas.edu” before logging in.

  1. Use your UT EID and password to log in to eProtocol at: https://spike.orsc.utexas.edu/fid (opens in new window). You will be directed to your Home page.
  2. When you arrive at your Home page, select the “Create/Open FID” button. The FID form will appear in a pop-up window.
    NOTE:
    If you are returning to a FID ID that you previously created and saved, you should see a created FID ID listed in preparation. Select the “Create/Open FID” button, then select “Edit” in response to the small pop-up window. The FID form will appear in a pop-up window.
  3. Begin completing your FID form in the pop-up window, starting with the Policy section. Mark the checkbox at the bottom of the Policy section.
  4. Use the “Next” or “Previous” button to navigate through the sections of the FID form. You can also go directly to the sections using the blue menu on the left side of the FID form.
  5. List the University research projects you are or will be involved in (regardless of funding or funding source) per the instructions in the Research section. To manually add a project, select “click here.” Mark the check box if you are not involved in research.
  6. Check the form for completeness and then submit the FID form by selecting the “Submit FID Form” button.
  7. You will receive a confirmation e-mail when the FID is successfully submitted and a second e-mail when the FID is filed.

NOTE: You cannot submit your FID form on the same day you complete your FCOI training because of the training record delay. You can edit and save your FID form, then return the following day to submit.

  1. Use your UT EID and password to log in to eProtocol at: https://spike.orsc.utexas.edu/fid (opens in new window). You will be directed to your Home page.
  2. You should see a created FID ID listed in preparation. Select the “Create/Open FID” button, then select “Edit” in response to the small pop-up window. The FID form will appear in a pop-up window.
  3. Use the “Next” or “Previous” button to navigate through the sections of the FID form. You can also go directly to the sections using the blue menu on the left side of the FID form.
  4. List the University research projects you are or will be involved in (regardless of funding or funding source). To manually add a project, select “click here.” Mark the check box if you are not involved in any research.
  5. Check the form for completeness and then submit the FID form by selecting the “Submit FID Form” button.
