For the Graduate and International Admissions Center (GIAC):

• Completed Social Work online application
• Official Transcripts
• GRE Score Report from ETS
• TOEFL-IBT or IELTS Score Report, if international student (if TOEFL-IBT or IELTS are not available then submit TOEFL and TSE)
• Statement of Financial Support, if international student

• Final field practicum evaluation (submit materials at mid-term if practicum is not complete)
• Case Application (download from education section of online application)
• Syllabi of Undergraduate Field Seminar and Field Practicum
• Resume
