For the Graduate and International Admissions Center (GIAC):
- Completed Social Work online application
- Official Transcripts
- GRE Score Report from ETS
- TOEFL-IBT or IELTS Score Report, if international student (if TOEFL-IBT or IELTS are not available then submit TOEFL and TSE)
- Statement of Financial Support, if international student

- Reference from faculty liaison, field coordinator or BSW Program Director (download from education section of online application)
- Final field practicum evaluation (submit materials at mid-term if practicum is not complete)
- Case Application (download from education section of online application)
- Syllabi of Undergraduate Field Seminar and Field Practicum
- Resume
