Students must fill out the following forms and mail them in to their Study Abroad Advisor as soon as their acceptance is confirmed.

• Student Information Sheet
• Release and Indemnification Agreement
• Health Clearance Form
• Authorization for Emergency Medical Treatment & Insurance Information
• Refund Policy
