- Fill out and attach this form to all materials that you send
to the Graduate & International Admissions Center. This will
help us match your documents to your application record.

Request for Reference and Statement of
Purpose forms
- Blank reference forms for you to give to the people who will
be providing your referrals. Don't forget to fill out the top
section with your information.

| Certification
of Financial Responsibility form
- Applicants from other countries must complete this form. While
this form is not required for consideration of your application
for admission, it is necessary for preparation of the immigration
document (Form I-20 or Form DS-2019) for the University of Texas
at Austin.

Senior Year Course Schedule
- Form to be submitted by international freshmen applicants who
have attended a Texas high school.

| Supplemental Information
for Graduate Program
- This is page three of the paper application. Some departments
request that applicants to their program who are using the online
application download this form to fill out and mail in. Other
departments have their own supplemental forms for applicants to
complete or have appended supplemental questions to the online
application. Check with the Graduate Advisor or Graduate Coordinator
of the department to which you are applying to find out which
form you should fill out.
