DHFS Alumni Distribution List Removal Request Form

We have established a series of Alumni distribution lists which can keep our former employees in touch with happenings and events at DHFS. If you would like to be removed from one or all of our distributions lists, please fill out this form and submit. If you are interested in updating your contact information or being added to our list services please refer to the DHFS Alumni Directory Form. Your removal request will go to the Human Resources office and we will expedite your request.
