Please follow the link below to fill out and submit a Banner Order Form.


Please note the following:

  • Please submit request at least 5 working days prior to Date Needed.
  • After receiving your request, the Marketing staff will design the banner based on your submission and send a proof for your approval, prior to actually printing the banner.
  • The list of approved logos can be found on the dhfs-shared server, under Banner Form and Logos - Standard DHFS Logos - Logos for Banners. Please copy and paste the name of the logo file into the appropriate section of the form. Please remember to always use DHFS logos that include Division of Student Affairs (DOSA).
  • For calculating the price, please locate and select the exact banner dimension and cost from the corresponding dropdown menu. The options are seperated based on Paper Type. If you need a double-sided banner, simply double that price and include in the Total Cost field. Otherwise, just enter the same amount that was found on the drop down list.
  • When you have completed the form and are ready to submit, you may wish to print a copy for your records. Look for the Printer icon to the far left of the Toolbar.
  • Do not forget to click on Submit Form when you are ready. If a Security Warning message pops up when submitting, please select Allow, in order to submit the form.
